If you need any correction in the Dual Nationality Certificate issued in favour of you, you may write and mail to the Embassy an application like the following:

Enclosures:
1. The original copy and a photocopy of the issued DNC.
2. 2 copies of recent passport-sized photo of the applicant.
3. Documents in support of the desired change.
4. Money order or cashier's check of US$22.00 (twenty two dollars only) payable to the 'Embassy of Bangladesh'.
N.B. If you send the application by mail, the application must be notarized. Even in case of walk-in submission, notarized application is preferred. Otherwise, the applicant must come in-person and sign the application before an Embassy officer.
