BASIC REQUIREMENT FOR VISA




  1. Please duly fill-in the attached fillable pdf Form. You need to fill-in all segments in Page-1, only the 'New Visa' segment in Page-2, the 'Checklist' in Page-3 and sign. In 'Payment Details' of page-2, the details of money order or cashier's check or official check have to be provided. No other mode of payment is accepted. Once filled-in save the document in your computer as pdf file, please print it and send/submit the hard copy to the Embassy with the required documents (visa supporting papers);
  2. If applicant is a defense personnel must submit an additional form;
  3. One color photograph of 2’’X2” or 45mmX35mm with white background taken within the last six months (attach using glue on the top left corner box of the application form. Do not staple photo);
  4. Attach existing valid passport (Original Passport) with a validity of at least six months, 2-blank visa pages in the passport and should not be Mutilated/damaged/tampered in any manner;
  5. Attach a Photocopy of the passport (both photo pages);
  6. If the applicant is a non-US national, s/he must submit a photocopy of his/her valid Green Card or work permit. US State ID or Driving License will not be considered as Proof of Residency;
